Fees:
An initial consultation/intake fee is $75 and sliding scale fees are offered for psychotherapy services: from $75 to $150 per one-hour session. Fees are determined by the client, based on ability to pay, and are due at the time of service. Form of payment is check, money order or cash. No credit cards or insurance are accepted. For clients wishing to use insurance, payment must be made by the client at the time of service and a statement will be provided monthly, for submission to their insurance company
Issues of Confidentiality:
All client records and information are treated with strict confidentiality. In the event of needed collateral contact (with physicians, prior therapists, attorneys, or representatives of the court system), a release of information will need to be signed by the therapy client to allow the sharing of specific information with specific individuals.
If the client is involved in a Child and Family Investigation or Parental Responsibility Evaluation that is ordered by the court, during a high conflict divorce situation, no confidentiality of records is available.
Telephone calls, messages, emergencies and crisis intervention:
If Dr. Milburn-Westfall is not able to take a phone call to his office, please leave a voice mail message. These messages are checked frequently and your call will be returned within 24 hours. If your call is a non-911 emergency, please call his cell phone: (303) 638-9975, and you will receive a prompt response. There are no additional charges for crisis intervention.
Appointments (cancellations and missed appointments)
When appointments have been made for therapy appointments, there is a 24-hour cancellation policy. Failure to cancel prior to 24 hours of the appointment or failure to appear for the appointment will result in being charged the hourly fee. In the event of severe illness, motor vehicle accident, or severe, inclement weather, no fee will be charged if the client calls with less than 24 hours notice, and the appointment will be rescheduled.
